The Cursillo Work Application form is designed for those who are already assigned to a team, and
need a printed form to give to the Rector/a.  If you want to sign up to be on a team in the future,
just go Back and use the Online Update Form,  and enter a date in the Want-to-Work field.
You should also use the Online Update Form to update your record.

To obtain a Work Application, we ask that you first update your record in the Cursillo Database.
Then submit this form, and you will receive a completed Work Application on your screen.
Just print the completed form, and mail it.

If you do not supply proper Birthdate, you will get a blank form.
